What to do if your blue badge has been lost or stolen

You can also download the replacement blue badge application form then 

• hand it into one of our Customer Service Centres, or
• post the application form and evidence to:

A £10.00 fee will be payable for all approved replacement Blue Badges. 

We will contact you to take payment once the replacement has been approved. 

Do not send payment when sending your application.
